Financial Considerations

There are many financial considerations to think about when moving into the community:

  • How will you pay the bills?
  • What will the bills be?
  • What services are you going to need to be successful in your move?

One very important detail you'll have to figure out is how you are going to get the furniture, dishes, pots and pans, as well as the other household items you will need for your own home. You can find some of these things in thrift stores, or perhaps friends and relatives will have some items for you.. Make a separate list of items you need for each individual room, where you'll get them, and an estimated or actual cost.

Nursing Home Transition Grant

While you are preparing to move from a nursing home, you need to be screened for a Community Access for Disability Inclusion, or CADI, waiver. This can be done while you are still in the nursing home. In addition, you may be able to access a one time grant of up to $3,000 for your transition costs.

There are limits on what the $3,000 can be spent on:

  • $1,000 for furniture
  • $300 for household items
  • $1,700 for moving services, delivery, damage deposit, application fee, mileage and labor

The county must preauthorize the grant, and the grant DOES NOT affect the other CADI waiver services such as home access, etc...
