Voluntary Meal Break Waiver Form

Access North provides an unpaid 30-minute meal break if you work more than six consecutive hours, in compliance with Minnesota law. If you wish to voluntary waive your right to this unpaid meal break, you need to fill out this waiver either daily or ongoing.

Extended Holiday Hours Request Form

If you are requesting increased hours to work on one of the standard Union Holidays that is over the daily authorized time, please follow the procedure and fill out the Extended Holiday Hours Request form.
